Player's Medical Form Injury Report Injury Log

Player's Medical Form

Player's Medical Form - Have all the player's medical forms completed by the first game of the season. Keep them with you in the zippered part of your trainer's bag.
At the end of your season, please return the completed Player's Medical Form to the player. Medical forms are confidential information and should never be kept by the trainer.

Injury Report

Injury Report -This form should be sent with the parents to the hospital (or dentist). Page 1 must be completed by a team official who witnessed the accident. This page requires the team official’s signature. Parents must ensure that Page 1 is completed in full before submitting (especially the Health Insurance section). Page 2 must be completed by a medical attendant i.e. physician, physiotherapist, dentist, etc. This form is for the insurance company, in case of a claim. Keep at least 2 printed forms in the zippered part of your trainer's bag.

Injury Log

This log should report each time a player has been -

removed for the remainder of the game due to an injury susutained during the game.
injured during a practice whether on or off the ice.
forced to leave a game or practice for unknown medical reasons.
injured during a hockey related event.

The Injury Log form, when filled in, gets put in the RMHA's Director of Discipline, Risk & Safety mail box at the arena.
